If you have a Mac, there's a third-party tool called "Gamma Control" by michelf.ca that lets you experiment with reducing the white point on your Mac's screen. (As a side-effect, blacks get crushed a little and colors get a little more saturated, but those are just side-effects.) Which means that you do have to increase the screen backlight brightness if you want an equivalent brightness level. Reduce white point adjusts the display's gamma curve so that pure white is darker. Both are similar but very expensive for what they offer though. If distractions are a real problem for you, you might consider investing in the full-size PDF e-reader Sony makes that aims at the legal and academic markets, or the ReMarkable tablet. I still think there's probably room for a textbook reader app that aims for "natural". You'll just need to take some time to get used to them and develop a good workflow. MarginNote and LiquidText (and to a lesser extent, Flexcil) try to give you digital ways of working that are as good, just different. There aren't any apps yet that let you manipulate long PDFs in exactly the same natural ways you'd read a book (sticking little post-its on the edges of pages in a way that is visible from other pages, folding page corners over, etc.). That being said, it doesn't feel totally "natural". If you find long-term reading tiring, consider turning on the "reduce white point" option in accessibility, which reduces overall display contrast.
